Hammertoes are a very common foot deformity. If you have one of these second, third or fourth toes shaped like a carpenter's favorite tool,hammertoe you know the pain, embarrassment, and impaired mobility hammertoes cause. However, this all-too-common problem can be solved, and your Stoughton and Madison, WI, podiatrist, Dr. Neal Katz, definitely can help.

The details on hammertoes

The Institute for Preventive Foot Health states that seven million Americans have one or more hammertoe deformities, and most sufferers are women. Unfortunately, women too often wear narrow-toed shoes with high heels which place tremendous pressure on the forefoot and toes. Obesity may contribute to this podiatric deformity as can bunions, a painful bump at the base of the big toe.

While hammertoes are not a sudden acute injury, but a problem which develops over time, careful treatment is required to:

  • Normalize the middle joint of the toe
  • Eliminate the corns and calluses which can form on the toe
  • Correct any ensuing gait problems and immobility of the toe
  • Make the toe look straight and attractive again
  • Use injection therapy (corticosteroids) to control inflammation and pain

Intervention

To determine the extent of a hammertoe problem, Dr. Katz performs a complete podiatric examination and X-ray imaging. Part of the examination will be hands-on so the doctor can see what positions and pressure points cause the symptoms. Based on his findings, Dr. Katz may advise:

  • A change in footwear to something roomier in the toe box
  • Shoe padding
  • In-office removal of any calluses and corns
  • Shoe orthotics or splints to relieve pressure and straighten the toe joint

Sometimes the deformity is so severe that surgery is the best treatment. Typically, your foot doctor advises surgery when the joints have stiffened severely or when more than one toe is affected. Also, age and how active a person is plays a role in treatment decisions.

BunionsWhat is a Bunion?

Are you dealing with a bunion? A bunion is a protrusion of the bone at the base of the big toe. While a bunion may seem like a bump, according to the (APMA) American Podiatric Medical Association a bunion is actually the enlargement of the joint at the base of the big toe – the metatarsophalangeal (MTP) joint. While bunions are a common foot disorder, it is not something that you should ignore as bunions can cause discomfort and become inflamed if left untreated.
 

What Causes Bunions?

Bunions can be hereditary and aggravated by the shoes you wear, especially high heels or shoes that don’t have enough room for your toes. Certain factors can also contribute to the development of bunions, such as if you have flat feet or low arches or if your feet pronate (when the ankles roll in towards each other during movement and cause excessive and prolonged pressure on the joints in the feet). If you are dealing with bunions, or think that you are, it’s important to seek help from a qualified podiatrist to get the care you need to relieve your pain and discomfort.
 

How a Podiatrist Can Help

Your podiatrist may recommend certain conservative at home steps you can take to minimize the discomfort. The first thing they may recommend is that you look at or change the kind of shoes you wear. It’s important to find shoes that are wide enough to accommodate your toes. Shoes such as high heels are likely to make the problem worse. Bunion pads can also help with your discomfort.
Severe bunion pain can restrict your mobility. Untreated bunions can continue to get worse if you don’t do something about them and can lead to other issues such as calluses and corns, or you may experience pain or redness on the site of the bunion, as well as swelling.
Other treatment options include orthotics or a combination of physical therapy and medication to relieve pressure and inflammation of the bunion. In severe cases, surgery may be needed to resolve the issue.

A child's feet grow rapidly during the first year, reaching almost half of their adult foot size. This is why podiatrists consider the first year to be the most important in the development of the feet. Proper care at a young age is essential for healthy development.  Since many adult foot ailments develop in childhood, periodic visits to your child’s podiatrist and basic foot care can help minimize these problems later in life.

A child’s feet are formed from soft, pliable cartilage which makes them more susceptible to deformities. A young child can be affected by foot conditions such as:

  • Intoeing
  • Out-toeing
  • Flat feet
  • Warts
  • Heel pain

Tips for Parents

Parents can help promote normal, healthy foot development for their baby.

  • Examine your baby’s feet regularly. If you detect anything unusual, contact your child’s pediatrician or podiatrist right away.
  • Encourage exercise.  Lying uncovered allows the baby to kick and move feet and toes freely so not to inhibit normal development.
  • Cover feet loosely. Tight clothing or covers restrict movement.
  • Alternate your baby’s position several times a day. Lying too long in one spot may place unnecessary strain on the feet and legs.

Growing Up

As your baby continues to grow and develop, so will the feet. It may be necessary to change shoe and sock size every few months, as tight-fitting footwear can aggravate pre-existing conditions. After your child takes their first steps, you should also carefully observe walking patterns. Intoeing, out-toeing, and gait abnormalities can be corrected when they are detected early.

A baby’s feet will carry them throughout life, so it’s important to begin good foot care at a young age. With our feet bearing the weight of our entire body, it’s no surprise that carrying excess weight may increase the chance of developing foot problems. In fact, recent studies have shown that overweight people experience more heel pain, tendonitis, arthritis, ball-of-foot pain, fractures and sprains in their feet and ankles than individuals at a normal, healthy weight.

Extra weight doesn’t have to be substantial to have an impact on your feet and ankles. As little as 10 or 20 pounds can trigger pain in the lower extremities. Being overweight changes the way your foot functions, and the force on the feet intensifies.

The most common foot problems from being overweight include:

  • Plantar Fasciitis: Excess weight adds strain to the plantar fascia, overusing and weakening it. This causes it to become inflamed and irritated. Heel pain is one of the most common problems caused by weight gain.

  • Tendonitis: When the feet endure extra weight, it eventually causes the tendons/ligaments to be overused, which leads to injury and inflammation.

  • Fallen Arches: An increase in body weight and pressure causes the supporting structures in your feet (muscles, tendons, and ligaments) to become stretched and weakened, breaking down over time. This can weaken the muscle which gives the foot its arch, causing over-pronation and leading to other problems such as knee and hip pain.

Other effects from carrying extra weight include changes in posture, changes in gait (steps become shorter), and stress fractures.

Losing extra pounds can help ease the pain and reduce problems caused by carrying excess body weight. Unfortunately, it's tough to lose weight when your feet hurt. To combat foot problems triggered by weight gain, ease into a low-impact activity that doesn’t require you to place pressure on your foot, such as water aerobics.  Always start any new workout routine slowly. Work with your physician to find healthy ways to modify your diet, and your podiatrist to select the best, most supportive footwear for your feet.

Foot pain is never normal, regardless of weight, as it indicates some type of stress or injury. You should always consult an experienced podiatrist if you are experiencing any pain in your foot.
